	2.	Read and sign our Service Provider Agreement Form which sets forth certain warranties/responsibilities of the broker as well as Eastern Savings Bank;
	3.	Supply copies of broker licenses for each state in which your company does business, Financial Statements for two years, resume(s) of Principal Officers, Articles of Incorporation, and a copy of the broker agreement used by your company.
