| Loan Submission Instructions |

Eastern Savings Bank requests that a Loan Submission Sheet be completed and submitted with all loan proposal requests. Please specify the terms requested including broker points, intended use of proceeds, and a brief summary of the loan proposal. In order for Eastern Savings Bank to provide a proposal decision as well as proposed terms (pricing), please supply the following documentation along with the Loan Submission Sheet: |

Complete 1003 (or alternative application form)-include the price paid and year acquired for the subject property, complete schedule of real estate owned, and itemization of debts to be paid |

Credit report or payment history on all guarantors |

| 3. |
Soft copy of appraisal (if available) |
|||
| 4. |
Copy of the signed sales contract/land contract for purchase money transactions |
|||
| 5. |
Rent roll/operating statements/Schedule E's/l120's for income-producing properties only |

| 6. |
Property photographs (commercial properties only) - refer to individual commercial loan program summary sheets for additional details on loan proposal submission requirements |

| It is Eastern Savings Bank's goal to provide a proposal decision and terms within 48 hours of receiving a complete loan submission package. Proposal terms are good for 30 days from the date quoted and are based on the information supplied by the applicant and/or broker. The Loan Proposal does not constitute a commitment to fund. The terms, conditions, and documentation requirements are subject to change as information is gathered and evaluated during the processing of the application. The Loan Proposal must be signed before Eastern Savings Bank will begin processing the loan application. The more accurate and detailed the information provided, the more likely your original loan proposal will be approved as submitted.
